Project Title & Description
Community-Based In-Home Asthma Environmental Education and Management
This project will enroll approximately 25-50 families to receive home visits as part of asthma care coordination services serving primarily the elementary child. Educational efforts will be geared toward empowering the family to increase skills to manage asthma and to decrease/remove asthma triggers in the home. A Home Environmental Assessment will be completed that evaluates the "general in-home environment" and will be used to identify potential triggers including identification of smokers, house dust mites, cockroaches, molds and animal dander. Information will be obtained on the room where the child sleeps. Parents and children will also identify triggers for asthma.
