A. REQUIREMENTS APPLICABLE TO ENTIRE FACILITY AND EQUIPMENT:

1. A permit is required to operate this facility.
3. The Air Pollution Control Officer may impose written conditions on any permit.
4 Commencing work or operation under a permit shall be deemed acceptance of all the conditions so specified.
5. Posting of the permit to operate is required on or near the equipment or as otherwise approved by the APCO/District.

10. Owner/Operator shall pay all applicable MDAQMD permit fees.
11. Owner/Operator shall pay all applicable MDAQMD Title V permit fees. (b) A permanent, conspicuous label summarizing the applicable operating requirements contained in Rule 1104. In lieu of a label, operating instructions may be posted near the degreaser where the operators can access the proper operating requirements of this rule.
